INTERNSHIP APPLICATION UNDER THE VOCATIONAL HIGH SCHOOL INTERNSHIP REGULATIONS
NOTE 1
According to the 2nd paragraph of the 12th article of the Internship Regulation of Burdur Mehmet Akif Ersoy University Vocational Schools, students who will do internships at the end of the second year must have taken all the courses taught in four semesters at least once and fulfilled the attendance requirement.
NOTE 2
Internship application documents will be submitted by mail or in person.
NOTE 3
Among the internship application documents, there must be a valid Very Dangerous Class, Class A, 16-Hour Occupational Health and Safety Certificate. Internship applications of students who do not present this document will not be accepted.
NOTE 4
Only the Appendix-2 Internship Acceptance Form requested during the internship application will be sent to your advisor via WhatsApp or email for advisor approval after being signed by the workplace.
After obtaining advisor approval, all documents must be sent to our School's Student Affairs Unit completely by 25.10.2026. If any of the documents are missing, the internship application will not be accepted.
NOTE 6
The internship duration has been determined as at least 20 working days.
B) INTERNSHIP APPLICATION DOCUMENTS
- Internship Application Document (Appendix-1):
This will be used to apply to the institution where the internship will be done. The document will be filled out after determining the internship dates before being submitted to the institution. - Internship Acceptance Form (Appendix-2):
After filling in the student information, it will be signed by the student, the institution official, and the advisor, and prepared in two copies. One copy will be submitted to our School's Student Affairs Unit, and the other will be submitted to the institution where the internship will be done. - OBIS Internship Application Form:
If the internship institution accepts, the necessary information will be entered into the internship information section in OBIS, and a printout of the form will be obtained. The process of entering internship information into OBIS is explained in detail in the attachment. - Eligibility Document:
The document showing health insurance will be obtained via e-Government. - Occupational Health and Safety Certificate:
Very dangerous class, Class A, 16-hour certificate. - Transcript Document
Note:
Workplace Wage Notification Form (Appendix-4) will be submitted at the end of the internship by students who do internships outside public institutions and receive wages from the institution where they interned.
Students receiving wages must also attach the bank receipt indicating that the payment has been made to the Appendix-4 document. The Appendix-4 document is not among the documents to be submitted to start the internship.
C) SUBMISSION OF INTERNSHIP APPLICATION DOCUMENTS
Students who complete their documents must submit the following documents completely by 25.10.2026 either by cargo or in person to our School's Student Affairs Unit before starting the internship:
- Internship Acceptance Form (Appendix-2)
- Health Insurance/Eligibility Document

After logging into OBIS, the options Course and Semester Transactions → Internship Application → Add will be followed, and the information on the screen will be filled in, and the Save and Print operations will be performed. The printed output will be added to the internship application documents. - Advisor Approved Transcript
- Occupational Health and Safety Certificate
Very dangerous class, Class A, 16-hour certificate. The document must be valid for the dates of the internship.
Applications with missing documents or those not made within the specified time will not be accepted, and the relevant students will not be able to start the internship.
D) FILLING AND SUBMISSION OF THE INTERNSHIP FILE
IMPORTANT!
- The pages where the work done in the file is written will be filled out daily, and the bottom of the pages will be stamped and signed by the institution official. A total of thirty pages will be filled out during the thirty working days of the internship. No pages will be skipped for holidays.
- The control checklist will be filled out and signed by the student.
- A daily internship attendance chart will be filled out, and the chart will have the stamp and signature of the institution official.
- A student photo will be attached to the cover of the file. A total of one photo will be attached to the file.
- The confidential evaluation form in the file will be filled out, stamped, and signed by the institution. The form will be placed in an envelope; the envelope will be sealed and stamped by the institution.
- The completed internship files must be submitted to our School's Student Affairs Unit within a maximum of 7 working days after the end of the internship. Complete files can also be sent by cargo.
